Written permission for Massage Treatment of children

Hi
THe FHT advocates the use of a letter when performing a massage on a minor. I am still a student and my friends neice wants to come with my friend (who is over 18) for a massage at the college where I am studying. Does anyone have a copy or sample letter that I could use so that her massage has proper consent?

This is my general informed consent form which is signed by everyone and anyone under sixteen needs to be accompanied by an adult who must both sign the consent form on the minors behalf and also stop and witness the full treatment.
Consent form:

I. Mr Paul Crick has done a consultation and he has explained both the nature and the form of treatment that I will be receiving which may vary depending on what is found once the treatment has commenced.

II. He has also explained that he is very happy to explain anything to me that I am not sure of.

III. Mr Crick has also explained that this consent form will cover any future treatments that I may wish to receive from him at any time in the future.

IV. By signing this form I am consenting to receive treatment from Mr Paul Crick.

Thanks Paul for the info. I notice that you're around Filey. We (family and I) spent quite a few weekends up in Staintondale - we have a caravan and thought it was lovely. Would be nice to pop in to say hello if we're around next year
