Reflex Consultation form

Hello fellow Reflexologists

Cany anybody let me have a copy of their reflex client record form? Not the main consultation form but the one where you keep of record of the reflexes etc. I really don't like mine but am struggling for time to do a new one[] and thought that one of my lovely friends would oblige??????????

Not finished mine yet, the consultation form is all done, but I am trying to find a good piccy to use. If you have the Louise Tucker Reflexology book there is a nice little foot piccy in the consultation form section which you may wish to include there. When I finally get around to it and I am pleased with the work produced, I'll post mine.

I just use foot/hand templates and write 't' 'c' etc on the relevant areas then write a narrative if required. It works for me!

I too just use a foot template which is half of the page of A4. The other half is blank where I wirte notes. I find that my treatment chnages withe very course I attend so if I did a detailed form it would soon be out of date!!

I can also post/send you one for you to perhaps to photo copy
. . but if you have a scanner draw a few templates . .honestly it doesnt take a s long as you think i have just changed all of mine & i didnthave a scanner buyt my neighbour did.
Sketch outline of all angles of the feet - dorsal plantar medial etc & then add them simply to a page with the list of reflexes & a key corns bunions etc - it shouldnt take long & it is is just as important to give time to your own needs as your clients. Once you have done them, your own designs to suit your own needs, you will have them forever - the thing is about having anothers is - honestly you will perhaps spend the same amount of time adapting them to suit your own preference as well as needs.

You have made me smile . I had a go not too long ago at drawing some feet and I nearly wet myself laughing at the results - pathetic! To think I come from an artistic family too. This must surely confirm that I am a changeling

When I qualified I was pointed in the direction of a printing company caled Currie International (01207 505191) who specialise in printing for therapists. They do a really good A5 card aimed at chiropodists which shows all angles of the feet with space for patient personal details then on the back are lines printed to record observations from each appointment.

I find them pretty good.

Hi Candie.

The British School have a good A5 card with a diagram of the feet on one side and space for client details on the other. All of the reflex areas are mapped out which is handy, but there are so many different charts that you would have to look and see if they matched yours.

Just wondering, we are using the reflexology chart which the AOR use, but I need to find a foot piccy to put on my treatment sheets. I can't find one that resembles the AOR one, could cope with just a piccy of a foot with skeleton, I can't decide and it's holding me up on my write ups.

Hi Candy,
Do you need a chart or a form?
I mean, I started off with one and dutifulyy filled it in for ewach client, but found that it took for ever. I know use a form for the health and consent details then a piece of A4 for each treatment. I find that my notes are more fluid and easier for me to interprit. Below is an example, of how I woudl write up a point.

EG L Ear: Hard/Crunchy, CLinet fliched any other information
any points previously observerd in teh londituduinal or tranverse
line.
Space

Next point: More details

This is not what I was tought at college, but find this help me compact information and make it easier to refer bactk to rather than looking and a diagram with lots of marks on.
