Parental consent form template

Hi there,

Im currently treating a 16 year old teenager, would she be classed as a minor?
Anyway if she is I need to provide a parental consent form to put in with my case study. We have covered GP letters in class but not parental consent forms.
Are there any templates available ?

My understanding is that at 16 she can provide her own consent. A minor (ie under 16) would need to be chaperoned as much for your protection as hers and you can cover the legal consent for massage by getting the client to sign the consent at the end of the consultation and then getting the parent / guardian to countersign.

At least, that is my understanding, anyone know differently?

Yes, as far as I am aware, so long as they are over 16, the consent of parent/guardian is not required. I use a separate consent form for those under 16 which is signed by parent/guardian and the person being treated signs the consultation form. However I see no reason why you can't do as suggested above and get parent/guardian to countersign the consulation form.
