Doctor consent letter? Parental Consent form?

Hi

I am doing the ITEC massage course. I have to create come forms and letter templates, I have got loads done now, but need to wite a letter to the doctor to confirm I can give a client treatement. Can any practicing masseurs help with wording on this, or any tips?

Also with a parental consent form?

Ms/Mr ---------------ID Card No------------, would like to have (this treatment)-----at our Saon and I would like you to certify if she/he is medically fit to undergo such treatment.

Oh now i have always been taught that you only give them a chance to say yes, not let them write comments
e.g. you reply slip should read something along the lines of:

I doctor ...................... give my consent for my patient ............... to receive treatment form .............
or i see no reason why my patient ..........should receive treatment.

if you give them the opportunity to say no then the chances are they will.
i have never had a refusal , but then the forms i send out don't offer the chance for that. if they refused they would have to write a separate letter to do it,,which quite frankly is too much like extra work for most g.p's

You are right MagicMum and we have discussed this at length before so suggest looking back through the various threads.

We do NOT need to get doctor's permission at all. Doctor's mostly know very little about complementary therapies so would not really be able to say whether or not the patient should have it or not. If you see what I mean!

oh i do its part of my code of conduct with my registered body and classed as serious misconduct if not done ,my tutor did say if that not possible then add disclaimer to bottom of fconsultation form and get client to sign.which i did when was a student.

Hi Funkymel. Are you with VTC/FHT? I think if you are then you will find that this is the big difference. They teach their students to steer clear of even the most minor complaints and refuse to cover them insurance wise if they disregard.

I trained with ITEC and it isn't the same. It's quite a big topic and if you look back on old threads you will see that we have discussed contraindications etc a great length.

There is nothing wrong with writing a doctor's letter if you so wish, and many therapists do, but it is the client's responsibility at the end of the day, and yes, you are right, get them to sign a disclaimer to cover yourself.

Most doctor's will not give permission if there is a serious health complaint and that leaves the client who (sometimes) desperately needs treatment left in limbo.
