Client/therapist policy statement

Hi Ya'all, well I am almost finished with school. Seven more school days left, but who's counting, lol.
I need help with a few final projects, I will post each request seperately.

I need to come up with a polycy statement, I would really like to see some examples of what others are using. Thanks a lot.

Here is the policy sheet that I have in my massage room:

Appointments:

I provide massage services by appointment only.

I cannot allow unattended children or pets to be present during an appointment. Please make arrangements prior to your appointment.

Cancellation/No Show Policy:

Kindly give at least 48 hours notice when canceling your appointment. Once your appointment is scheduled all other requests for that appointment are refused.

You will be charged up to the full amount of the missed session in the event of a late cancellation or no show. Gift certificates holders will forfeit their certificate in the event of a late cancellation or no show.

Late Arrivals:

If you arrive late to your scheduled appointment your appointment end time will remain the same. Everyone's time is valuable and the session will end on the scheduled time and the full amount bill be due.

If you are running more than 15 minutes late for a half hour appointment, the appointment will need to be rescheduled and you will be responsible for payment for the current appointment.

If you are running more than 30 minutes late for a hour appointment, the appointment will need to be rescheduled and you will be responsible for payment of the current appointment.

No other appointments will be made until any balance is paid in full.

Minors:

Any child under the age of 18 must be accompanied by his or her parent/legal guardian.

Professionalism:

I am a professional massage therapist. Comments or actions of a sexual nature will not be tolerated.

I reserve the right to refuse services to anyone that is intoxicated, suffering from a coneforum.xxxious illness or for any other reason that I feel is necessary.

Returned Checks:
In the event of a returned check, there will be a fee of $30.

These are the policies listed on my intake form that all clients have to sign off on.

Please read and check off the following:

• I understand that I need to cancel my appointment at least 48 hours (2 days)
  in-advance of my scheduled appointment or I will be charged for the missed
  appointment. Follow-up appointments will not be scheduled until balance is paid.
  
  I understand that the massage I receive is provided for the relief for muscular tension
  and adhesions, not as a substitution for medical treatment.
  
  I understand that it is my responsibility to update the massage therapist with any
  health-related matters and that I have presently answered the above questions to the
  best of my knowledge. I will not find the therapist at fault for any problem resulting
  from the lack of information by my part.
  
  I understand that any illicit or sexually suggestive remarks or advances made by
  me will result in immediate termination of the session, and I will still be responsible
  for payment of the scheduled appointment.
  
  I understand that if I am late arriving to my appointment, my appointment time will
  not be adjusted. My appointment will end at the original time.
