Several fellow practitioners have inquired about the block on my Massage Client Intake form that features the letters M I S P A, so rather than repeating the answer individually, here it is for all that are interested:
The form is based on one used at a ladies only day spa in the U.S. where I first worked as a Masseur (don't ask me how long ago that was....it makes feel "old"). This was well before much of the knowledge of massage, licensing, certifications and professional memberships available today. Still, even then, we were tought to be careful of CONTRAINDICATIONS.
M I S P A (pronounced as "My Spa") is a memory aid for practitioners of the issues they need to discuss with the client prior to starting any body work, and the letters represent the following;
M = Medications
I = Injuries
S = Surgeries
P = Pregnancy
A = Allergies
Marking through each letter on the form once the item was discussed created a record for liability protection. If anyone has a more up-to-date and more comprehensive memory aid, I would be interested in it please.
