I am thinking of offering a 10 minute back and shoulder or head and shoulder massage at lunchtime for a week where I work to raise money for a charity and charging £5.00 per person
There won't be time for people to fill in detailed consultation forms so I was thinking of putting the following notice up
Please note massage is not suitable in this situation[/b]for people who suffer from [/b]
diabetes, cancer[/b] [/b]Varicose veins, or history of thrombosis, Infectious skin diseases, acute back pain, high blood pressure, or any heart disease [/b]
Or are pregnant [/b]
as a full consultation would be necessary prior to treatment[/b]
[/b]
have I left out anything?
Also i will ask them to sign a disclaimer as I think that it is professional to do so and was thinking of using the following
I understand thatthis is a "taster session" to raise money for name of charity
I confirm that thereare no health reasons why I should not have this treatment and therefore give my permission for treatment
