Advice please

I am thinking of offering a 10 minute back and shoulder or head and shoulder massage at lunchtime for a week where I work to raise money for a charity and charging £5.00 per person
There won't be time for people to fill in detailed consultation forms so I was thinking of putting the following notice up
Please note massage is not suitable in this situation[/b]for people who suffer from [/b]
diabetes, cancer[/b] [/b]Varicose veins, or history of thrombosis, Infectious skin diseases, acute back pain, high blood pressure, or any heart disease [/b]
Or are pregnant [/b]
as a full consultation would be necessary prior to treatment[/b]
[/b]
have I left out anything?
Also i will ask them to sign a disclaimer as I think that it is professional to do so and was thinking of using the following

I understand thatthis is a "taster session" to raise money for name of charity
I confirm that thereare no health reasons why I should not have this treatment and therefore give my permission for treatment

Therapist[/b]


have I covered all the bases?

Hi Soar,

I'm doing the same thing but not until early next year. Everything you've mentioned looks good! I'm not completely organised yet but I did think I would type out all contraindications on a page and laminate itto show each client and ask if any of the contraindications apply to them. Then on the short consultation form make a mention that all contraindications to massage have been explained and then add the disclaimer as you mention.

I probably won't get around to doing much more before Christmas but If you are doing yours in 2007 as well I would be happy to share whatever I come up with. Just let me know.

Does the disclaimer include contact info, like name address, email, phone number, too? It can be a great way of building up a mailing list if you need to.

Thank you all really good advice here for me to work on tho after the day i've had I'm not inclined to do it this side of xmas
funny how you can start the week full of enthusiasm and then feel "why bother" !!
